Medication Administration |

Please remember that if your student is to take any medication at school, including over the counter meds, I must have a doctor's prescription. This includes motrin, tylenol, tums, and cough medicine. I would be glad to assist in faxing the required documents to your provider. Medication must be in original container. The link for the medication form is below. Call and ask to talk to Nurse Cynthia if you have any questions!

Cough drops will be permitted without a physician signature under the following guidelines:
1. Written parental/guardian permission is required. Please see me for a release form, or send in an note saying your child has permission to have cough drops at school.
2. The parent must supply the cough drops.
3. The nurse will dispense the cough drops.
4. Candy, such as Jolly Ranchers or peppermints, cannot be substituted for the cough drops.

Yes, your child may miss PE without a provider's note. Parents may send in a note asking that their child miss PE for up to three days. After three days I will need a note from the doctor. It needs to tell me how long your child may miss PE.

No, I cannot give your child any medicine you might send in (eye drops, cough medicine, tums) if I do not have a provider's order. I'd be glad, though, to help you obtain one.

No, I can't tell you if anyone in your child's class has strep throat, chicken pox or head lice. This is a privacy issue.

Yes, I have extra clothing in my office. Now that the weather has turned cold, we do have extra coats. If your child comes home wearing borrowed clothing, please launder and return to school. If you have items you wish to contribute, please send in labelled "To the Nurse".

Illinois Health Requirements |

All the paperwork that is required for school can be quite confusing. Listed below are the required exams and who needs them!
- 1. Physical/immunization exam - this is required of all pre-k, kindergarten and students new to Illinois. For our school district, these must be turned in by the last Friday in September. If you move to our district after school has started, you have 30 days to complete.
5th graders must show proof of the Hepatitis B immunizations.
Students entering sixth and ninth grades will be required to provide proof of Tdap vaccination.
- Eye exam - this is required of kindergarten and students new to Illinois. A vision screening by your MD, DO, PA or nurse practitioner does not count.
- Dental exam - this is required for kindergarten, second grade and sixth grade. This is to be turned in by 15 May 2013.
